Andrew Harding, Emergency Department Pharmacist at Austin Health, spoke to us about the main issues facing Emergency Departments when it comes to the issue of readmissions and guidelines that help reduce readmissions rate.

Viki Brummel, Hunter New England Health, Network Manager, Aged Care and Rehabilitation Services Clinical Network, explains to us the pros and cons of developing the Transitional Aged Care Program, challenges and successes faced during service development and the issues faced from a client/carer perspective.

Helen Fithall, Manager, Ambulatory, Continuing Care & Allied Health of Austin Health, spoke to us about her thoughts on the current status of unnecessary emergency department visits, how the Residential Outreach Service work and what have been the programme's major challenges and key successes.
